Please complete in full and submit the following information to Ivy Lender. Items 1 – 6 in this checklist are required to begin the underwriting process.

☐ Articles of Organization / Incorporation for Borrower Entity
☐ Operating Agreement for Borrower Entity
☐ Borrower Entity Tax Identification Number (EIN)
☐ Purchase Agreement (only for acquisition loans)
☐ Preliminary Title
☐ Title Officer Contact Information
☐ Verification of Funds – 3 Months of Bank Statements
☐ Evidence of Property Insurance
